REQUEST FOR PROPOSAL
Americans with Disabilities Act (ADA) Self-Evaluation
 
                                                                             
The City of Medford Public Works Department is soliciting proposals from qualified firms to perform and compile data for a comprehensive Federal Americans with Disabilities Act (ADA) Self-Evaluation for street intersections and sidewalks within the public right-of-way in the City of Medford.  Proposals will be received by Alex Georgevitch, P.E., Deputy Public Works Director/City Engineer for the City of Medford, 200 S. Ivy Street, 2nd Floor, Medford, Oregon 97501, until 1:00 p.m., local time, April 24, 2018.  FACSIMILE PROPOSALS ARE NOT ACCEPTABLE.
 
As soon thereafter as possible, the proposals will be reviewed and evaluated by a City of Medford Selection Committee.
 
DESCRIPTION:  This Request for Proposal (RFP) seeks to secure the most qualified consultant to perform field investigations within the City right-of-way facilities in order to identify obstacles limiting accessibility, describe methods to improve accessibility, and suggest a schedule for achieving ADA compliance that can be used in the preparation of the Citys ADA Transition Plan.  The selected consultant shall have relevant experience with all federal accessibility regulations, public agency provided services, programs and activities requirements, and preparation of accessibility survey and evaluation reports for a Title II entity.
 
The Request for Proposal (RFP) may be obtained by contacting the Engineering Division via email at publicworks@cityofmedford.org or by calling (541) 774-2100.  Instructions will be provided on how to obtain the RFP electronically.  The RFP documents include Sections I through V more particularly described as follows:

DESCRIPTION:   The City of Medford Parks, Recreation and Facilities Department is seeking qualified contractors to bid on installation of new concrete needed to provide accessibility and connectivity to picnic tables, benches and pathways at Jackson Park. Contactor is responsible to provide all labor and materials necessary to successfully meet project specifications with budgeted funding.  Bid will be awarded to lowest qualified bidder within project funding.

DESCRIPTION:   This project construction will occur on a 3 acre parcel located on the west side of Lone Oak Drive between Shamrock Drive and Michael Park Drive in Medford, Oregon. Contractor will provide all labor, equipment and materials required for the construction of Village Center Park, to include: concrete sidewalks, interior pathways, pathway and sidewalk lighting systems, picnic shelters, playground elements, sport court, pre-fabricated restroom building, drainage system, landscaping and irrigation.
CONTRACTOR shall implement a CITY approved soil protection plan to ensure soils under turf and landscape areas are not damaged.  Soil shall not be compacted when the soils contain moisture. 
 
There will be a MANDATORY pre-bid conference and walk-through at 2:00 P.M. local time, on April 26th, 2018.  Interested bidders must be present to be eligible to submit a bid, though subcontractor presence is not required. The pre-bid conference will be held at the Santo Community Center, 701 N Columbus Ave, Medford, OR 97504. Immediately after, an optional on-site walk-through will occur at Village Center Park, Medford, Oregon.
