This project is located on five (5) street sections listed below, within the City of Medford and Jackson County, Oregon.

This project includes all labor, equipment, and materials necessary to construct the following:
 
The project includes a Type III Micro-Surfacing as specified; on five (5) arterial street sections in the City of Medford, to total approximately 42,779 S.Y. Included in the scope of work is all vegetation/debris removal from cracks, crack seal, paint removal as specified, traffic control, public notification, sweeping, and prep work to mask and restore all manholes, monument cases, valve boxes, storm grates and restore gutter-line to pre-work condition. The City’s field representatives shall determine the exact limits and requirements of each street. Start Date: August 5, 2014. Completion date: Sep 5, 2014. Estimated cost Range: $200,000- $300,000. Class of Work: Asphalt Concrete Paving and Oiling.

Bids must be accompanied by a certified check equivalent to 10% of the proposal payable to the City of Medford, to guarantee that if a proposal is accepted, a contract will be entered into and its performance secured. A Bid Bond to like effect and amount with a corporate surety will be acceptable for this project. Bids must be in writing and signed by or on behalf of the bidders.
 
All Bidders shall be prequalified with the City of Medford, as provided by law under Oregon Revised Statutes, at least 5 days prior to opening of bids.
 
All of the provisions of Section 279C.800 through 279C.870 ORS, as amended by Senate Bill 477, relating to wage rates to be paid on all contracts for Public Works in this state must be complied with and the statement attesting to the contractor's willingness to do so must be signed and submitted with the bid.
 
All bidders must state at the appropriate place in the bid documents, whether or not the bidder is a resident as defined in ORS 279A.120. A percentage increase, as determined by the Department of General Services, will be added to nonresident's bids for the purpose of determining the lowest qualified bidder.

Hawthorne Park Renovation

Sealed proposals for Design/Build construction of the Hawthorne Park Renovation will be received for the City of Medford Urban Renewal Agency, Oregon, by Jennifer Habermann, Purchasing Agent, City of Medford, 411 W 8th Street, Room 380, Medford, Oregon 97501, until 2:00 P.M. local time, on May 6, 2014. This project is a Design/Build alternative procurement process and is Exempt from Competitive Bidding pursuant to the criteria set forth in Medford Municipal Code 2.563(B) allowing for alternative contracting methods as described in ORS 279C.335, according to state law. This project will be awarded based on a competitive selection process, but the award will not be based solely on low price quote and rather to the Most Advantageous Proposer to the MURA. Respondents may comment on any specifications which they feel limit competition.

There will be a MANDATORY pre-proposal conference and walk-through at 1:30 PM on April 16, 2014. Interested parties must be present to be eligible to submit a proposal. The pre-proposal conference will be held at the Red Cross conference room, 60 Hawthorne St, Medford, OR 97504. Immediately following the meeting, the site walk-through gathering point is at the Hawthorne St. parking lot in the northeast corner of Hawthorne Park, Medford, Oregon. 97501.

DESCRIPTION:
The Medford Urban Renewal Agency is seeking a design and build firm for the renovation of Hawthorne Park, including the design and construction of (1) replacement and expansion of the parking lot on Hawthorne Street, (2) a dog park north of the existing Main Street parking lot, (3) a playground/plaza area in the northeast section of the park with a restroom, shelter and play elements, including a water play feature and (4) miscellaneous landscape and irrigation areas. The park renovations are guided conceptually by the adopted Master Plan and Conditional Use Permit. The approved Conditional Use Permit (C.U.P.) is attached to the RFP as Appendix 1. The cost for the complete project is not to exceed $900,000.00 and shall include, but not be limited to design, permits, equipment, construction, and contingencies. This project will require a Guaranteed Maximum Price (GMP) for Design-Build of the Hawthorne Park Restoration Project.

Proposals must be accompanied by a Bid Security as prescribed in the Instructions to Respondents, payable to the City of Medford Urban Renewal Agency, Oregon, in an amount not less than ten (10) percent of the amount of the Proposal. The successful Respondent will be required to furnish the necessary additional bond(s) for the faithful performance of the Contract, as prescribed in the Instructions to Respondents.

In accordance with ORS 279B.100 the MURA may cancel the procurement or reject any Proposal not in compliance with all prescribed public Proposal procedures and requirements, or may reject for good cause, any or all Proposals upon a finding of the MURA that it is in the public interest to do so.

Proposals received after the time established for receiving proposals will not be considered. Respondents on this work must comply with Federal Construction Labor Standards and with all other applicable federal governmental requirements including, but not limited to, equal employment opportunity actions as specified.

No Proposal for a construction contract shall be received or considered unless the Respondent is registered with the Construction Contractors Board or licensed by the State Landscape Contractors Board as required by ORS 671.530.

All Respondents must state at the appropriate place in the Proposal documents, whether or not the Respondent is a resident as defined in ORS 279A.120. This project includes all materials, labor, and equipment necessary to brush, scarify, water, and grade approximately thirteen- and–a-half (11.8) miles of local "unimproved" alleys in sections "A" through "G" of the City of Medford’s alley system entirely- in Summer 2014. Included in the scope of work is vegetation/debris removal from the alleys to a nominal minimum of 13 feet in width and 10 feet in height; scarifying the alley surface to a minimum of 3" below finish grade; application of water simultaneously with the scarification; and grading the scarified surface back to a proper longitudinal slope and cross-section. All collected debris and vegetative material disposal is the contractor’s responsibility and is considered to be incidental to the work. The City’s field representative shall determine the exact limits and requirements of each alley.

Each Proposal must be submitted on the prescribed form and must be in writing and signed by or on behalf of the bidders.

To be eligible to bid, the Bidder must submit a fully executed Contractor's Prequalification Form, as outlined in the Invitation to Bid, and be pre-qualified.

Prequalification submittals by interested Bidders must be received by the City Engineer at least five (5) calendar days prior to the bid opening date. Interested bidders must be pre-qualified before submitting a bid. Prequalification application forms may be obtained at the office of the City Engineer located at the Engineering Division offices which on the second floor of the Medford City Hall Lausmann Annex, 200 South Ivy Street, Medford, Oregon, 97501 (541) 774-2100 or may be obtained on the City of Medford website at www.ci.medford.or.us. The executed Prequalification Form shall be addressed to the City Engineer at the specified address.
